PRIVACY PROTECTION AND INFORMED CONSENT

 


You have the right to know about the procedures and products that are to be used in your participation in this clinical research study so as to afford you an opportunity to make the decision whether or not to join the study after knowing the risks and hazards involved. You also have the right to privacy and the expectation that none of your medical or personal information will be divulged during or after the study is completed.

The INFORMED CONSENT DOCUMENT is an effort to make you better informed so that you may give or withhold your consent to participate in this clinical research study. You will have all of the time you need to study this document and decide if you want to take part in the clinical study.

Your privacy protection is important to us, and we will take every effort to keep you and any of your medical and personal information private and secure. When you join the trial you will be given a 4-digit personal number so that your identity and data will be hidden to the investigators in the study. You will also set up a personal passcode so that using your patient 4-digit number and passcode you can enter data on your signs and symptoms and know that these data will be kept confidential.

If you forget your 4-digit personal patient number or your passcode, they can be recovered. Contact the Research Associate (Dr. Russell) for your 4-digit patient number. Your password or passcode can be recovered or changed at any time.

You have the right to know the following:

1. What personal information of yours is collected for the trial.
2. How this information is used.
3. With whom is this information shared, if anyone.
4. What security procedures are in place regarding collection and storage of data so that personal information will not be released without your permission.
5. How can any inaccuracies be corrected in your personal information.

The Institute for Molecular Medicine is responsible for the collection of the online data from this clinical study, and we would like you to know that we comply with all regulations of the National Institutes of Health with regards to privacy.
